Could mean a multitude of things. Torture, becoming a walker, sex with a pretty woman, anything that you consider worse than death, really.
A fate worse than death...
It describes a experience/event that is very unpleasant, painful or just flat out horrible that it can't be compared to death itself.
Being in a situation where death would be preferable.
Like, Crawford attaching you to a pole via your rectum and leaving you there as a warning.
