That is a fan-made model. A guy at another forum tried to implement it into the game and it didn't work. Wanna know why? BECAUSE IT'S FAN MADE!

If this was real, Lee cut off his hand higher than where it's cut off in the fan made model.
Telltale would take lee's season 1 model and convert it to a season 2 model and edit it. And I'm pretty sure that fan made model is a season 1 model
Not only does Lee not have a realistic sized beard, he's dead for sure. I don't get how Matt can even consider this being possible...

Oh dear, I am deeply affected by this, and will deal with this sadness by not caring.
Not only does it not make any sense realistically for Lee to survive and find them, but it would destroy to game mechanics. How could you bring back season 1's protagonist who has a largely determinant personality? Would be a mind-fuck and would simply not work, although imagining Lee with scruffy hair and a blade for an arm gets me giddy with geekiness.
Yeah I imagine that if telltale is going to throw in a dream scene, it'd show Lee as a zombie or with a bullet in his head and not him all happy and alive.
To be honest they will probably have him blaming Clementine because that is the part of her that feels terrible about what happened and thinks because she ran away she basically killed Lee.
