On my first playthrough of season 1 I didn't die at all. I died a few times on my second playthrough, and in season 2 on Ps3 there is bad lag coming out of cut scenes so I generally always die at the start of each scene.
Never with Clementine, thankfully.
My Lees've died a bunch of times, particularly when replaying Episode 2.

As Clem, I was killed a couple times in the shed. One item doesn't seem to allow Clem to live after you pick it up. Other than that, as Lee, my controller at the time liked to move forwards when I didn't, so he died several times in the Brenda encounter.
I just thought of something, I REALLY feel bad for the people who died when Lee was a badass walking a road of Walkers and killing them to get to Clem, imagine how awkward it woulda been

IMO having two hands was better for me, he squeeze the shard of glass as the blood dripped down and the music started like he risked his life saving a kid he didn't really know about but he had a close relationship over one year, and his hand was dead anyways so why not use it for the best of it?

I too have died to Brenda. I didn't realize the speech icon had come up so I kept walking. Can't remember any other times though. Haven't died as Clem yet thankfully. I watched a death montage on YouTube. How can people stand to see her die over and over again? A part of me dies with every death.
I'm fairly sure that I was given the option to grab the rake while I was still on the ground, and when I did, I was given a mash button session, but it autofailed me even when I was mashing faster and harder than when I choked a bitch out with one arm. And it was way less satisfying.
I died like 5 times with Lee. And twice so far with Clem, first was the QTE dodging walkers while escaping Winston and second time was when I fell of the bridge in episode 2.
A little embarrassing but i died a shit load when the walker sneaks up on you in the shed.I mean the fight was easy,what was killing me was the killing blow when you push em' on that sharp board thing.I couldn't find the interaction so i was killed again again. #Still.Not.Bitten
I HATE failing QTE's with Clem. So far I've only failed twice; once while fighting Sam (horrifying) and once when I failed to grab Matthew's knife in time and she got disemboweled. (even more horrifying). Compared to Lee, I take great pains to ensure Clem doesn't become food.
