I do not think anything is wrong with Kenny.
A lot of people say that because of how generally happy-go-lucky Kenny is in episode two. Looking back at epsiodes four and five of season one. You can tell Kenny was terribly depressed, he wanted to find a boat, to get to a safer place, and when that failed, he basically gave up. After he attempted to sacrifice himself, and survived, and met Sarita, the couple FOUND a safe place, with plentiful food, power, and even a goddamn Christmas Tree. Kenny probably thought that he would never see any of these things again. Then Clementine came (with Carver following) and fucked everything up for him. But also, Kenny got to see a little girl that he thought was long dead.

There is something definitely up with him. His willingness to kill when Carver showed up, and his seeming ENJOYMENT of it make him look even crazier.
He's unhinged, but we probably won't see just how messed up he is until the next episode.
As for Sarita, is Kenny not allowed to be in a relationship? Not sure why it has to be assumed that he is trying to replace Katjaa.

Now if Kenny continues to call Clem Duck and starts to treat her as his dead son, then starts to call Sarita Katjaa, then I would say Kenny's lost his marbles, but as of right now, it just appears it was a momentary lapse of memory which would be perfectly normal.
It's also the zombie apocalypse. Who's going to really be sane two years in after seeing family and friends turn or be torn to pieces? If you're sane, you'll end up dead. If you're off your rocker, you're going to end up dead. If you teeter between sanity and insanity, you're going to end up dead. Does it matter if Kenny's lost it or not? He'll likely end up dead by episode 5 anyway. I very much doubt Telltale will save him for a third season as much as I would like to see him make it. Question comes down to whether another human kills him or a walker ... or he commits suicide.
