Mentally unstable? I don't know if I would go that far, I work part time in a psyche ward and all I can say is Lilly didn't strike me as that mentally unstable, even when she shot Carly/Doug. She was grieving her father and under a lot of stress but I don't think it mental instability.
That's later on. He immediately tries to murder Lee after Lee saved his life in the first Episode, and he doesn't get around to making up for that until several months later. That ain't right in my book.

Lilly had a very big-sisterly attitude towards Clementine. I think if they reunited Clementine might be a little standoffish, though I'd like to think all the times Lilly was supportive and kind to Clementine would outweigh those last moments.
I think Lilly was overwhelmed by her position of authority and all of the loss she experienced. If she was taking care of Clementine by herself or just with Lee, she would be very capable and very dedicated, kind of on Christa's level.
Actually, that was the moment I really started to respect Larry. I mean, it was the perfect opportunity to let Lee die with no risk, reward or effort on Larry's part, yet he still tried to save him.
