Did Telltale change original episode plans?

Well, we all know that Telltale changes the plot around based on how we respond to the characters. Like in Season 1, on the Episode 3 slide we see Duck smiling sticking his head out of the train even though in the actual episode Duck is bitten and completely immobile after getting on the train. The general premise is there, but the actual scene in the episode may be completely different.
However, everyone was expecting Rebecca to go into labour in Episode 3 because that is what we see on the Episode 3 slide (or at least Clementine sitting by Rebecca while she is in a dark place unable to move). I think that Telltale wanted for us to have to decide to kill Rebecca or not after the labour, giving us a super hard decision. Like we all hated Larry and Duck, but when the time came to end them, it wasn't easy for everyone to do it.
But in Episode 2 and 3, Rebecca is really nice and changes and takes a liking to Clementine, so the player would become much less likely to want to kill Rebecca. I think that is why they removed her child birth from the episode. I think originally after the camp is attacked by walkers Clementine and Rebecca would hide inside while she is in labour, and the episode would end there. Then, episode 4 would begin with her escaping and wiping the blood on herself. But episode 3 ends with the blood scene and no labour which is instead in episode 4. So the title slides basically switch.
I think in Episode 4 we will be faced with a really tough decision as to whether save Rebecca while she is in labour or another member while the group is attacked. Or maybe Telltale thinks the labour is a good way to explain what happened to Christa, so Christa will return in Ep4 and assist Rebecca in labour. Maybe Christa will deliver the baby and want to keep it because she feels like it is so similar to the one she lost herself? And we must decide to side with either Christa or Rebecca?
I think the child birth scene is going to be HUGE and much much more important which is why they moved it from Ep3 to Ep4.
Right now, I am getting used to Rebecca because I see now she has been through a lot and I am going to help her in Episode 4 anyway I can, but if I have to chose between her or Christa, I will choose Christa.

I think that is normal, when we originally plan something, it's just a shell of what it trully is. When you are actually developing the scene, you think about the details and stuff like that, so everything changes

I read the question wrong, I'd choose Christa if I have to pick one to save.
People are often known to look too much into things. Look at people who think they see faces in burnt toast or in clouds. We want to see something that really isn't there. Just because Clem was holding Rebecca didn't mean she was in labor. We just wanted to see that. Maybe she tried to commit suicide in the original plan for the episode so Carver didn't get her and the baby. Women tend to cut their wrists when trying to commit suicide so if she tried that, there would be no blood on her upper chest and face. If the birth of the baby is in episode 4 or even 5, I actually hope it isn't done cheesy. I'd rather they not show it if they're not going to make it realistic (i.e. too much drama). They have Rebecca screaming during the birth, that's just going to draw walkers which would just make the better answer to leave her screaming and run away (thus sacrificing Rebecca and her baby). But of course, Hollywood has conditioned people to think childbirth is nothing but a scream fest for the mother if she doesn't have an epidural and that the father must run to get hot water so I can't help but think that's where Telltale's going to go. I'd honestly rather have Rebecca in a closed room with maybe 1-2 other characters and Clem outside with the rest watching for walkers or survivors from Carver's group. Only reason for Clem to be in with Rebecca to deliver the baby would be if Clem was the only one around, the adults felt she had to know how to deliver a baby in case she had to deliver another going forward, or she had to deliver her own. Honestly, neither are really good reasons because women can deliver their own children naturally just by trusting their bodies. They've been doing it for thousands of years. Only reason to have another woman around is for support or in the odd case something goes wrong. A tween for support might be better than nothing since Sarita's really the only other woman and she's walker bait now. Carlos is dead. Alvin's dead. I doubt Rebecca want's Kenny, Luke, or Nick near her while she's in labor. Jane hasn't been in the group long enough for Rebecca to trust. And then there's Bonnie who Rebecca knew before. So Bonnie's a far better choice to be with Rebecca for the birth than Clem. If Christa helped her, unless Rebecca's in dire straits and there's absolutely no one else, I don't see Rebecca telling Christa help her. Being in the same situation at different points still doesn't remove the fact that childbirth is a vulnerable time for a woman. How many want a stranger between their legs while they're trying to push a baby out? Clem could say Christa's her friend, but a pregnant woman is not likely to trust some random person when she's in pain.
As much as I'd like to see Christa back, it might be better to have Clem see her upon arriving at their final destination in episode five and the game ending on that much like we saw the two people on the hill at the end of season one. It'd be a lead into season three. End the season the way it started ... with Christa and on a more happy note.
