I looked carefully at the thumbnail and I notice that's was not doug... maybe telltale made a character to look like doug or maybe its doug brother.

It's a placeholder damn it, how don't you get it?!
It's Doug. It has some sort of symbolic meaning from the looks of it, since Carley is on the other side of the thumbnail. One theory that has some possibility is that it's representing a 'Carley/Doug' situation that's going to happen... possibly... or even that it's just representing people that Clementine blames herself for the death of, seeing as how she feels responsible for a lot of things. Who knows, let's just wait and see.
