When Arvo accusing Clem's group of stealing he is talking about the meds, even if you returned them. I think Jane had him pegged. He was a junkie and that was why he was hiding the med bag and creating a fake sob story about a sick sister.. He probably stole them from his own group. After you return them, he pins the disappearance on Clem's group anyway.

I think it might be because Jane took his gun or he's just being a dick. I played one of my Clem's (who didn't take the meds) silent through the encounter and Arvo said something along the lines of, "It is a shame. You were very nice."

I am more concerned with why did he try to hide the bag with the meds in the trash can on the observation deck. I have a feeling he was trying to appeal to the other Russians, not only because they are from the same nationality, moreover to increase his odds of survival by proving himself useful to the others.
