Black Velvetopia (Psychonauts)
Some Borderlands 2 levels pop up in my mind. The "Hunting the Firehawk" is one, the map is really cool and the music/atmosphere to it tops it off. Another is going after the bunker, and the final mission. Those are some crazy ass levels, non stop action. And the very first one of course, really fun level to the start of a great game.
Fort Frolic, Hospital Pavillion : Bioshock : Creepy as fuck. Steinman and Cohen are both terrifying and darkly hilarious.
Fontaine Futuristics : Bioshock 2: Alex the Great is one of my favorite villains of all time. "Contract terminated! Benefits rescinded! Fired! Fired! FIRED!"
Zombie Island : Borderlands : Especially the part with the rocket.
All of Solstheim : Dragonborn
All Ghillied Up : Call of Duty 4 : People forget how intense and cinematic this level was, especially for the time.
Gravemind : Halo 2 : Fighting your way through the convenant capital during a huge civil war was pretty awesome.
All the heist in GTA V, Four Leaf Clover in GTA IV, The Casino Heist in San Andreas.
Some other good ones are The Once and Future Slab and A Train to Catch (mainly because of Tiny Tina).
I like the one where you have to sacrifice the midget for a cult that worships Lillith.
That whole thing is one of my favorite side missions in the game, along with this.

Hollow Bastion (Kingdom Hearts 1)
The tainted Mountain (Dragon's Dogma)
Stratus City (Ratchet And Clank)
That's all I got for now.
