Who would you save?
The bad guys break into Wayne Manor and stabs Alfred while throwing a bomb out of the window. If you save Alfred Gothom City blows up. If you decide to save the city, Alfred dies. Would you save the many and let a close friend die?

Obviously I'd save Gotham. I don't really think Alfred will be able to live with himself knowing that his life costed the lives of millions in Gotham City. And I don't think Bruce would be able to live with it either.

If you ask me, I would obviously save Gotham for many evident reasons. However, since I am Batman, can not I save both? I bet I can.

Option 3: Bruce calls Kent on speed dial and Superman swoops in quickly to get the bomb out of Gotham and into space.
Considering how incompetent the characters tend to be despite their powers. I wouldn't be surprised if Superman failed to do that. Or even realize.
Superman decides to walk instead of fly or something stupid like that.

Allowing many to die in order to save one is so unforgivably selfish, Batman may be a conflicted vigilante, but he isn't Kenny.

I would always prioritize the lives of the people I know and love over the lives of complete strangers, no matter how many strangers we're talking about. Quality over quantity. However, in the example given in the OP, I would definitely pick the city - because by saving Alfred I would not just be dooming thousands or even millions of strangers, but also all of my other friends and loved ones. But, had the question been "Would you save your own children [I know Batman doesn't have any children in this game, it's just a hypothetical] if it meant killing every other person in GC?" then my answer would have been YES! Without hesitation.
Save Gotham. I know Alfred, he would never forgive Bruce (or himself) if Gotham was destroyed for him to live.
It's really the same situation as Max and Chloe in Life is Strange where you have to choose between the town and Chloe. Alfred is like your father, sure, but Bruce would know that he wouldn't want to have Gotham destroyed instead of him. Bruce would be devastated sure, but he'd inwardly know he had made the right choice.
I do wonder if we'll have a Dark Knight and have to choose between Harvey and Vicki at some point, mind.

"Batman must always save Gotham." I believe Alfred himself said something like that at the end of Arkham City, when Bruce was contemplating saving Talia al Ghul from The Joker instead.
Although ... if it were Alfred's neck on the line, he might change his tune. "Batman must always save Gotham, but Bruce Wayne must always save the man who raised him, cleans his humongous house, and makes him breakfast in bed. Choose wisely, ingrate!" lol
Alfred would want Batman to save Gotham
Despite the ridiculous scenario (one random bomb tossed from Wayne Manor window destroys the entire city, yet misses the manor from where it came?), in this case I'd safe Gotham. I couldn't put one life ahead of many, no matter how important they are to me. I mean, how selfish would one have to be to allow so many people to die for the sake of one person, and thus allowing that person to live with that immense survivor's guilt? It was messed up in LiS, it'd be messed up here too.

This is a "Save Chloe or Arcadia Bay" decision. However, I would prefer Batman save Gotham.

I'm still trying to figure out why a hand-held bomb that has the capability of blowing up an entire city for some reason wouldn't harm the mansion Batman and Alfred are in because it was thrown out of the window.
For this reason, I'd stop the bomb. Cause y'know, Alfred and Batman would be nuclear toast anyway.
