The concept was dropped during the silver age and then re-popularized by the Dark Knight Returns as mentioned above. Several comic writers took to reuse the concept for a few years till it was dropped because it became hard to explain why Batman was still not killing and even harder to explain why they weren't dead when realistically no medical help could reach several of them in time.
Frank Miller himself kept exaggerating what humans could survive in his further books till we eventually got his fully psychotic 'goddamn' Batman who spoke like a angsty teenager about how awesome he was and was deluded enough to think he didn't kill people by smashing into them with his car, mutilating them and just leaving them lying there, drugging them and dropping them in lakes or setting them on f-king fire. He was also deranged and facist, told his recently kidnapped 12 year old Robin to eat rats and had implied incest with both his mother and Alfred.
He also wrote the atrocious Dark Knight 2 years later, where Batman visually kills people. And was the one who popularized the throw them into live electricy 'knockout'.
But before that insanity he also wrote the masterpiece Batman Year One which was a huge inspiration for telltale, to the point its the closest Batman story the plot resembles even though its still incredibly different. So I guess they wanted to invoke more Frank Miller by bringing his 'knockouts' back. Don't be surprised if even more insane 'no way he should be alive' moments happen in the future.
I'm pretty sure Batman was aiming for the head, but it just hit him in the eye. His friend looked like she was about to be gunned down, I don't blame him for not taking the extra step of being careful.

Yeah, again. Its based on the FANTASTIC works done by Frank Miller such as Year One and DKR which is another reason to like this Batman. But sadly it apparently also came with Franks need to exaggerate violence for no reason.

Getting hit in the eye with something sharp isn't going to kill a person unless it passes through the eye and into the brain. If it's just the eye, despite being a serious injury it is totally survivable. That was true even centuries ago, which is why eye patches were a thing.
