i'm pretty sure she goes if you tell her she should go with you and not tell her to rest in the cave. but i'm not sure
She goes to Soren's fortress with Axel and Magnus (or maybe Olivia and Ellegaard? Not sure what happens if Magnus dies) if you save her in ep 1 and tell her to keep fighting.
Axel goes to the fortress no matter what with Magnus/Ellegaard.

Yes, you must save Gabriel in Episode 1.
Not necessarily always with Ellegaard, in my playthrough Ellegaard stayed with my sick Gabriel
