Tomb of Sammun-Mak "Failed to initialize Direct3D"

Trying to run Tomb of Sammun-Mak, and keep getting the error "Failed to initialize Direct3D". Running Windows 7 home premium 64-bit. Have updated my graphics drivers, DirectX, applied all Windows updates that I'd missed, rebooted, re-installed Sammun-Mak, and still no luck. Tried disabling virus checker. Tried running at XP service pack 2 compatibility (it complains it can't find the data files, oddly) and Vista service pack 2 (same error).

Penal Zone did, and continues to, run fine.

Full dxdiag.txt wouldn't attach, so trimmed out parts that didn't seem important, attached what was left. I wish I was joking about the first half of that sentence.

Ideas, anyone?

    I had hoped they'd fixed this for episode 2, but obviously they didn't...

    I just re-installed Windows due to hardware upgrades and it happens to me without having much else installed, so it shouldn't be that hard to reproduce:

    Windows 7 x64 + ATI Radeon 5xxx + ATI Catalyst 10.4 + Second Screen + Launching Episode 1 or 2 = BOOM!
