when the narrator decides not to kill max he says max has finally made an unselfish act. what is he talking about exactly? you never see monster max do anything of this sort

Sam's in-game notebook says they're individual aspects of Max's personality. At least Sam doesn't consider them to be the real Max -- on their own. But the "Save Sybil" sequence is the only time (other than their general goal of putting the populace to sleep) that all the Max spores share and communicate the same sentiment. For all intents and purposes, the only aspect of Max's essence this decision seems to leave out of the loop is his Superego.
So yeah, I think it counts as Max "as a whole" deciding to save Sybil under his own willpower.
And he DID survive! DUN DUN DUNNN!!!
