Chapter 1: Georgia's First City
Savannah was indeed Georgia’s first city.
Chapter 2: Down by the River
Some lyrics from Neil Young – Down by the river. Maybe a death of a woman in this chapter? Or perhaps the boat escape idea gets shot down? Because there are no boats down by the river.

The chapter I think Kenny begins to break down and gives up hope. Lee is supportive.
Chapter 4: Bedside Manor
The chapter where the group discovers our demented hospital that we have seen in the trailer.
Chapter 5: Georgia's Last City
Perhaps Savannah was Georgia’s first city build, and last to fall?
Chapter 6: For Whom The Bell Tolls

Sexual themes? Or day after a disaster within the group?

The definition to Penultimate is “Next to the last”
Chapter 1: Georgia's First City
The actors meet. She wants to "thank" Lee for saving her.
Chapter 2: Down by the River
She goes down on him.
Chapter 3: Support Group
Another woman joins.
Chapter 4: Bedside Manor
She introduces Lee to a friend, whom seems eager to "thank" the hero as well.
Chapter 5: Georgia's Last City
Lee bones her.
Chapter 6: For Whom The Bell Tolls

Chapter 7: The Morning After
They have a quicky. Then she takes the morning after pill.
Chapter 8: Penultimate
They wrap up the story. Lee has been thoroughly thanked.
That bit cracked me up.
On topic, It seems the wiki has had all the chapters/achievements for quite some time now. They've got the supposed achievements for Episode 5 as well. Don't know if it's a reputable source but I'm assuming most of 'em are correct.

Chapter 1: Georgia's First City
They first go into the city most likely meet another group or residence of Georgia.
Chapter 2: Down by the River
Most likely Lee and the group will help out in this new place and new group. Trying to put their past behind them.
Chapter 3: Support Group
The girl with the axe will appear.
Chapter 4: Bedside Manor
The picture is a hospital symbol so most likely the girl who you meet will tell you about that place and judging from the video in the trailer its not the best place to go while sick.
Chapter 5: Georgia's Last City
Lee's group confront Georgia's group and shit goes downhill fast, Lee's group vs. the new group.
Chapter 6: For Whom The Bell Tolls
The bells are rings for Lee's group death, they go to the church, and i sense this is where we lose someone.
Chapter 7: The Morning After
After loosing someone or multiple people, they long for morning and a brighter day.
Chapter 8: Penultimate
Judging from the picture it seems that they are overrun by zombies. Most likely will have to escape the horde below in Georgia and possibly something will happen.
I've known these since I bought the season pass,all the PS3 trophy titles were there from the beginning.Telltale did release a patch to change one or two for episode 2 and quite a lot for episode 3,episodes 4 and 5 have stayed the same.
They arrive on the train, to see the city is pretty wrecked and continue down the street.
Chapter 2: Down by the River
After quiet abit of talking and wondering around, they find the docks, no boat in sight, they look around for a place to stay.
Chapter 3: Support Group
The group could lose hope?
Chapter 4: Bedside Manor
You found the place to stay, omid gets left behind? or maybe walkie talkie guy locks you in this place like in the trailer.
Chapter 5: Georgia's Last City
The horde of zombies following the train arrives.
Chapter 6: For Whom The Bell Tolls
The churchbell begins ringing, bringing all of the zombies to it L4D style, you rush in to get whoever was ringing it out? a couple/one person dies.
Chapter 7: The Morning After
This is the morning after the loss happened, everyones trying to cope.
Chapter 8: Penultimate
No clue about this, telltale will surprise us.
(Chapter 1) Arrived in Savannah
(Chapter 2) Head to the boat (& don't find it)
(Chapter 3) Have to shack up with a new group as a matter of urgency (probably in Crawford because...)
(Chapter 4) ...you're in the medical centre and videotape trouble is imminent
(Chapter 5) You decide to leave
(Chapter 6) On the way out of the city the church bells ring, the walkers show up and all hell breaks loose.
(Chapter 7) Probably just talking over a new plan, not sure what the petrol can is for, burning a body?
(Chapter 8) More walkers, moving on and more trouble
Obviously there's a lot to fill in there. Logically I can fill in most gaps apart from how Lee ends up on his own in a fight against Ninjagirl.
Okay. Not to be a douche or anything, but "Ask not for whom the bell tolls...." is from John Dunne's Devotions upon Emergent Occasions. Generally speaking, when the church bell would ring, it was a signal that someone in a village or community had died. This is basically saying that sooner or later, the bell will toll for thee - or, you're going to die. Deal with it.

After that, you find a completely new group in Support Group.
Oh, and in The Morning After, Lee gets bit.
You also get a chance to kill or keep Ben in the group.
