I played it twice one with my arm and one w/o it. If you kept your arm you would begin passing out more throughout the game, though the difference of the ending doesn't change.
I kept the arm because I thought Lee would be too weak if he lost a bunch of blood, and he needed to focus on Clementine, since he was dead anyway. It turns out that if you keep his arm, he basically uses it for nothing (other than sticking a piece of glass in it and ignoring it during the zombie fight). Many of the animations look as though they are done without the arm in mind. So ... there seems to be no real difference.

Same for me, but in the badass scene where Lee's going down the street full of zombies you get to stab zombies with a metal scrap and the cleaver.
