There's no real answer for this and as far as I know, Kirkman said the cause of the outbreak will never be truly explained.
But I think it was in the series, the doctor in CDC said "This is what takes us down, this is our extinction event". I really liked that explanation and I interpreted it as it was just nature's doing. We humans like to think that we're so special that we're above it's laws, but we're not. So when the nature decides it's time for us to go, it finds a way. And just like that, we're no longer at the top of the food chain.
The comic was launched on the premise that it was caused by aliens as a way to soften up the planet for conquest. That was a bullshit story cooked up to get the initial funding. Then later Kirkman was asked, "So when do the aliens get introduced to the story?"
The TV show talks about a contagion being carried by everyone in the last episode of Season 1.
I think Ben did it, like someone else said. He was taking the car into Macon one day, when he realized he forgot t put on his seatbelt. While the car was going down the road, he tried to put it on while driving, and the car veered into the oncoming lane. At that moment, a truck full of hazardous chemicals was about to strike him, but veered into the ditch to avoid killing him. The chemicals spilled everywhere, and the truck driver was the first walker.
