I think they just call him the doctor for the group in the apocolypse but he really wasn't one in the normal world....

    He most likely is, and it's becoming tiresome to hear this.

    The bite thing was a developer oversight, not a part of his character. There is literally no evidence that he is not a doctor.

    He can't tell the difference between a dog bite and a walker bite.
