Save file malfunction?

In my episode 2 Alvin was killed. I'm 100% sure of this.
Last night I played the beginning of episode 3, up to the point of being brought to the camp and introduced to the new guy. (I forgot his name, kinda feminine, only 1 arm.) However, I had a lot on my mind so I didn't really get the most out of it.
So tonight I restarted the episode, so that I could pay better attention and enjoy it more. However, Alvin is ALIVE!! Does anyone know why this or what could of happened?
In my play through last night, he was dead. Tonight, he's alive? It's the SAME game file too. I've made sure.
I'm really nervous my other decisions have been affected too..

Edit - I play on the 360

    When you played Ep 2, did you wait all the way until the end before exiting? If you didn't, then it may be generating choices for the last chapter of Ep 2 for you instead of doing what you chose.

    I'm pretty sure I did. Plus last night, the game had my decisions correct. I don't know why things are messed up tonight.
