Arvo Theory and Episode 5 Predictions (SPOILERS)
Thanks to Juice_Box for giving good details to support this theory. (Sorry Juicy, for some reason I can't link your profile here but I'll tell you about this anyways).
THEORY: Alright, so we know that Arvo was was trying to hide the medicine and then claim that it was for his sister when held at gunpoint. Why would he have to hide the medicine though if it was for his sister? He's obviously lying about something. He could be lying about having a sister, but I think he's lying about the medicine being for his sister. What's his motive though? Why lie? Was it to save his own life? No, there's more to it than just that.
I think Arvo is pulling a Ben situation. Remember how in Season One, Ben had to sneak medicine in a bag and bring it to a vent on the outer wall of the motor inn for the bandits to collect? What if Arvo is doing the same thing? At the end, we see Arvo and his group surround Clem's group, but what if they aren't the real threat? What if there is something bigger out there, like a much bigger group with more weapons? That's why he attempted to make the lie about his sister so convincing, because he knew that not putting the medicine in there would put his group and danger, and most likely his sister.
PREDICTION: Okay, prediction time. The group is now in a shootout but we have no idea who has been shot. I predict that Clem will be one of the few to get shot. It won't be fatal though, she'll most likely be shot somewhere in the stomach or shoulder. This is where Arvo tells his group to stop firing (Assuming he's the leader) upon noticing that Clem has been shot. He obviously feels terrible for what he's done, so that's when he brings Clem and her group to their holdout (Which is possibly in the town and NOT in the forest like he previously mentioned. That's probably a lie).
This will cause them to use some of their medicine on Clementine and get her patched up. In the end of this conflict, they end up realizing that it was a misunderstanding and this is when Arvo explains to them about the dangerous group and how Clem/Jane put them all in danger. By the end of the episode, or somewhere in the middle, this is when we'll be introduced to the group. The REAL threat. They'll be heavily armed obviously because assuming that Arvo has a decent sized group, they'll be in fear of a group that's much larger than them.
This can be a similar situation of what happened in Episode 3 of Season 1, and once the talking has been taken care of, that's when the shootout happens. This is the season finale after all so something big has to happen, and this big event might as well be a battle between these two groups. People will die obviously, but if Telltale really wants to hit us hard with something, they'll kill off a major character in the midst of the battle. After all that has been settled, the group sets out to go find Wellington, which is where Season 2 could possibly end off (Basically confirming a Season 3).
POSSIBILITIES:
These aren't part of the theory or prediction, but rather just things that had crossed my mind. What if the town that our group is heading towards IS Wellington? The group doesn't exactly know how far they are from Wellington. They probably don't even know what it looks like, but this kills the prediction I have.
What if Winston's group has a part in all of this? What if they are part of Arvo's group, or part of the larger threat group and have taken Christa in? It's possible that we'll see Christa in No Going Back. This leads me to wonder where Victor is in all this.
This is far fetched and the least likely to happen, but character like Vernon, Molly, Lilly, Eddie or Nate can be involved in this somehow. Possibly in Arvo's group or the larger group. We'll most likely see them in Season 3.

Arvo isn't the leader of the group obviously.
There are syringes; that could mean a lot of things, but I'm sure they're junkies.
I believe Arvo is genuinely "good," but hooked in with the wrong people. He's a bit of an errand boy/translator without any actual power. Also, there are achievements viewable on the wiki for any curious people.
I'd also like to point out you're suggesting something a little similar to the Saviors of the comics and the show.
Yeah, that all sister thing looked really strange to me, I mean if she is so sick why would he go to a deserted place like that instead of bringing the meds to her. He is definitely hidind something...
But I don't think Clem will get shot, she has the advandage of being small and everyone will aim up during the gunfire not down (except if one of them shoots her in a cold blood).
How is getting shot in the stomach not fatal?

The only thing that is certain here is that Luke will be gone again after the shooting is over.
Well, what's he gonna do if he gets shot? Crawl away at a rapid pace?

The Walking Dead will close the loop in Episode 5: No Time Left. Count on it.
Clementine will survive the shootout unscathed. She was on the sidelines.
Bonnie will die. She was a character that had a gun trained on her and her gun on a Russian.
Mike will die for the same reason Bonnie dies.
These characters die so that Telltale can focus on Clementine and Kenny/ Luke.
Kenny will (1) Survive, if Clementine shot Rebecca. Kenny therefore would not be distracted from the standoff (2) Be wounded, but not die immediately, if Clementine calls for help, since Kenny was distracted to save the baby.
Luke will be fatally injured, since he was distracted by Kenny or Clementine shooting Rebecca. He will probably not die at the beginning of the episode.
Either way, the last episode Telltale will make for Walking Dead, is No Time Left. Expect Clementine and the baby to be the sole survivors and make it to Wellington, I am Legend style. Expect whoever is left with Clementine to sacrifice themselves heroically so Clementine can live. Their last words will be a cliched, "You are the only one who deserves to survive in this world gone to sh*t. So live."
