Clem didn't have to shoot. (204 Ending)
In the end of Amid the Ruins, the group is surrounded by Russians. However, Rebecca is seen turned as a walker, and probably going to eat the baby. Clementine has a choice, either [Shoot Rebecca] or [Call for Help]. As many of you prevented the baby meal, the shooting could have been avoided. Remember the screwdriver Clem had? What about the nail file? Clementine could have just finished her off quietly instead of shooting. There was the less noisy, but still noisy side of calling for help, but it just attracted the attention of others to shoot Rebecca, what would lead to gunfire.
Think of the lives we could have saved. Oh man, I just can't wait for Episode 5. There's No Going Back from the gunfire anyway, but the silent kill would have been the best choice.

What caused the Russians to shoot was when that idiot Luke shouted, "NO!" And they would have shot Clem if she moved too much because they would have seen her as a threat, even though she was just going to stab Rebecca.
As gimmeseason2 pointed out, if Clem moved quickly to stab Rebecca in the head rather than shoot her, that could have set the Russians off just the same way as her (or Kenny) shooting Rebecca. Quick movements can be just as bad as a gun going off. How do they know she's not going to dive for cover while the others shot at them or go grab something that's out of sight? After all, they knew Clem technically attacked Arvo with Jane. Even though they got a laugh out of Clem being a child, for all they knew, Clem was the mastermind and the group took their cues from her. After all, Jane, an adult, didn't have to listen to Clem if Clem didn't steal from Arvo. She could have told Arvo to go to hell and took the bag herself regardless of what Clem wanted.
If Clem were standing right next to Rebecca, she could have done it quickly and quietly, but one of the Russians (Arvo in particular) might have alerted the others.
There's also the fact that you'd think everyone would be on high alert for walker noises which Rebecca did make before she was shot. Even if no one did anything, the noises she was making would have started in something. If not that, then the screams of the baby being eaten would have.
No matter how you look at the situation, it was going to lead to a gunfight.
Along with the two points brought up above, she really was in a "heat of the moment" there were guns pointed right at her and she just saw a woman carrying a baby turn right in front of her.
What caused them to shoot was Clem or Kenny firing their gun (killing walker Rebecca) but it wasn't intended to start a shooting. Luke yelling "NO!" was because 1. Clem or Kenny shot Rebecca and/or 2. He knew a gunshot would have caused the Russians to start shooting too.
Yeah because obviously somebody shouting NO after hearing a random gunshot is the major factor leasing into a gunfight
i think Luke would of took a hit maybe a fatal hit,because he looked away from where the guns were being pointed and was more concerned for Rebeccas state.Kenny is used to handling guns so maybe a hit but he will survive,Mike im guessing dies,and Bonnie.
