I believe that Clem was forced by the Ranch group to torture someone ,with the knife seen in the flash back, in a slow and drawn out matter to earn her keep in the group or they would harm aj. She also during her days their had to witness various ways of painful interrogation. Hence the phrase that aj said, " before the blood."
