Besides the dozens of times I (sometimes intentionally) died in Jurassic Park, and other times in games, there's one distinct memory I have, in the Sons of Winter, there's a bit where Gared has to crawl underneath a cart or else he gets shot by Frostfinger, and I died there a lot. More than 5 times I'd bet.
When playing Minecraft Story Mode Season 2 there's a part where you're suppose to build a diamond pick ax to mine into a wall. I decided to build a sword for shits and giggles and ended up getting shot and floated into the lava ceiling by the weird mob things. That was pretty hilarious to see
Adding to my post before I died in story mode season 2 episode one a lot while swimming up the waterfall in the Admin Temple. And about a month ago I finally got around to playing GOT and I died so many times it not even funny it me because I suck at PC controls I can’t go straight from Q and the arrow keys then go straight to the mouse I need everything to be close.
A couple of times in the Walking Dead Season One. Once when taking out one of the Walkers near the RV at the motel in A New Day and again when climbing the stairs in Around Every Corner (while you were in Crawford) I also died during 400 Days when you're running through that cornfield in Bonnie's story. I don't actually think I died at all in Season Two, and I definitely didn't at all during Season Three.
As for other Telltale games I also died in Tales from the Borderlands episode 2, I was taking too long trying to stop Vasquez from shooting Rhys and Vaughn.
EDIT: I actually just remembered a very recent one, I died during Batman Enemy Within when solving Riddler's puzzle with the faces. Even though it was pretty easy I did mess it up.
I crowd-played Batman S1 with my friends and in Episode 1, Catwoman falls off of the roof and you need to grab her hand so she doesn't plummet to her death. I, of course, purposely didn't grab her to see what happened and she just fell without a sound and Batman just somberly nods his head in shame. I wish I had a video cause it absolutely killed me.
Batman S2 Ep1's puzzle lockers after looking at the monitors from Riddler's chamber was pretty annoying (especially since the wheel in the chamber was buggy and I couldn't rotate it twice). I died in those two scenes a few times.

My controller disconnected from my computer, which made me fail a QTE.
I've died a couple times, but not in any notable ways. I think the most recent time was the swimming scene in Minecraft Season 2 Ep 1.

Plenty of times. Sometimes I like to take my time and figure things out, I don't fare so well with those times QTEs.
If you die in a Telltale game....you will get a phone call...telling you that you will die in seven days...true story.
