I shot Felix back in episode 1 and he died but Sasha mentioned that i let him get away. Did my choices not carry over properly?

Nah, they didn't carry over properly probably. I don't remember that happening to me and I killed him.
No it carried over fine for me. Maybe you lost progress
She talked about him as if he was alive but later asked me what I told him when I let him explode. Kinda weird.
